How do I request copies of my tax forms (1099s, 1098s)? To obtain copies of your forms for tax purposes, please call our Member Contact Center at 647-1111 (or toll-free at 1-888-786-2791). 8. What is St. Mary's routing number? St. Mary's routing number is 011400149. 9. How do I report a lost or stolen ATM card or VisaŽ Debit Card? To report a lost or stolen ATM card or VisaŽ Debit Card during normal business hours, you may call the St. Mary's Member Contact Center at 1-888-786-2791 or 647-1111. If you need to report a lost or stolen VisaŽ Debit Card after normal business hours, contact the Visa Hot Card Center at 1-800-754-4128 and have your VisaŽ Debit Card Number at hand. The Visa Hot Card Center will only be able to identify you and service you by your VisaŽ Debit Card number. Having your card number ready will assist in the servicing of your lost or stolen card and expedite the process. If you do not know your card number, please contact the St. Mary's Member Contact Center during business hours at 1-888-786-2791 or 647-1111. You should refer to your Truth-In-Savings and/or Electronic Funds Transfer (EFT) brochures, given to you when you received your card, to determine your liability. |
